Courier Authorisation

  • Courier service is available for card or certificate delivery within Singapore
  • Standard delivery is $30 per trip, excluding Tuas and secured zones
  • Tuas and secured zones such as Airport zones, Ports, Shipyards, Sentosa, Checkpoints, Camps, Supply Bases, Jurong Island, ALPS Ave, SATS will be charged at $40 per trip
  • Email the completed form to courier@sg-akc.com after payment

Authorization Letter

  • Authorisation Letter is required before sending a representative to collect a card or certificate
  • Course registered under company: companies may authorise a representative to collect on their behalf
  • Course registered under individual: individuals may authorise someone else to collect their documents
  • Unauthorised personnel will not be allowed to collect cards or certificates
  • Please take note that with effect from 1st April 2011, all cards and certificates have to be self-collected for all courses.

GIRO Facilities Evaluation Form

  • Use this form to apply for GIRO facilities evaluation
  • Complete the form together with the Interbank GIRO application where required
  • Mail the completed documents to AKC’s Finance Department

  • Use this form to apply for Interbank GIRO payment arrangement
  • Submit it together with the GIRO Facilities Evaluation Form where applicable
  • Completed forms should be mailed to AKC’s Finance Department

Letter of Appeal/Review of Assessment

  • For participants who wish to appeal or review their assessment result
  • Applicable when the result is Fail or Not Yet Competent
  • Submit the completed appeal form and appeal fee at AKC’s Customer Service Counters
  • Appeal outcomes will be communicated within 14 days by email or phone
